- Provide clinical pharmacy services including medication therapy management, drug information, and therapeutic drug monitoring.
- Participate in multidisciplinary rounds and provide recommendations for optimal medication therapy.
- Conduct hands-on sterile compounding and ensure compliance with USP <797> standards.
- Check and verify medication orders for accuracy and appropriateness.
- Collaborate with healthcare teams to ensure safe and effective medication use.
- Document clinical interventions and patient outcomes.
- Assist with medication reconciliation and discharge counseling as needed.
- Minimum of 2 years of experience working as a clinical pharmacist in an inpatient setting.
- Active pharmacist license in the state of Louisiana.
- Proficiency in EPIC EMR system preferred.
- Strong clinical pharmacy skills including therapeutic drug monitoring and medication management.
- Excellent communication and interpersonal skills.
- Day shifts, working 8-10 hour shifts between 8:00 am to 8:30 pm.

Summary:
A hospital in Shreveport, LA is currently seeking a dedicated Clinical/Inpatient Pharmacist to join their team for an 8-week assignment, starting on 5/20 and ending on 7/13. There is potential for the contract to be extended. This position offers day shifts with flexibility between 8:00 am to 8:30 pm, working 8-10 hour shifts. The ideal candidate will have a minimum of 2 years of experience working as a clinical pharmacist and proficiency in EPIC EMR.
