Description
We are looking for experienced team members who believe in the hospice philosophy and are ready to work closely with our patients, their families, and the staffResponsibilities:
- Report patient care/condition/progress to the patient's physician and Clinical Manager continuously.
- Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems.
- Prepare appropriate medical documentation on all patients, including any case conferences, patient contacts, medication order changes, recertifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other health care disciplines in providing optimum patient care. We presently use
Kinser as our EMR. - Confer with the physician in developing the initial plan of treatment based on the physician's orders and initial patient assessment. Provide handson care, management, and evaluation of the care plan and teaching of the patient per physician orders, under the Clinical Manager's supervision. Revise plan in consultation with physician based on ongoing assessments and as required by policy/regulation.
- Administer ongoing care and case management for each patient, and provide necessary followup as directed by the Clinical Manager.
- Provide admission, case management, and followup skilled nursing visits for hospice patients.
- Participates in weekly team meetings and case conferences, as necessary, to ensure appropriate care and service are provided to the patient.
- Exhibits an attitude that promotes harmony and goodwill in the workplace.
- Promotes quality, comprehensive services through a team approach.
- Performs other duties as assigned.
Education and Experience:
- Must have current or compact RN licensure in the state of Texas.
Hospice Nurse Certification (CHPN) is required:
- Valid Driver's License and Automobile Insurance. Pass the Background check.
- At least
four years of experience, preferred, as a
Hospice field Nurse in a home hospice setting
Skills and Abilities:
- Shall have a genuine interest in geriatrics, preferably, shall have experience in geriatric care.
- Must be able to assess, evaluate, and communicate effectively.
- Must demonstrate sound knowledge and actions in patient care and decisionmaking.
- CPR certification is required, and other medical certifications are highly encouraged (i
e:
PICC line, IV therapy, chemotherapy, etc.)
- Participates in continuing education to broaden knowledge and acquire skills necessary to be effective in all aspects of job performance.
- Must have reliable transportation and a valid driver's license.
- Able to fulfill shared oncall requirements with immediate 24/7 availability when assigned.
