Description
Full time position available for a Patient Navigator who is passionate about serving the community-at-large.Patient Navigator will provide referrals and linkage to integrated healthcare services (SUD treatment, mental health services, primary care, HIV care/prevention services, etc.) at TTC as well as referrals to supportive services.
Candidate will be a member of a multidisciplinary team of behavioral health professionals.Duties include but are not limited to conducting intakes, providing internal and external referrals, serving as a patient liaison within the agency and with outside entities in order to facilitate in the resolution of any issues that interfere in the client's ability to remain in medical care; maintains accurate and confidential patient records; using company vehicle drives team to and from partnering sites.
Ideal candidate will have a good driving record, a California driver's license, at minimum a bachelor's degree in related field, and/or one year of related field experience.
Good communication, organizational and task management skills are essential. Bilingual English/Spanish is a plus.
