Description

The Non Medical Case Manager reports to the Director of Social Work and provides non medical case management services and care coordination for Park West patients as instructed.


Duties Include:

  • Receive referral from Director of Social Work.
  • Complete intake and schedule initial evaluations.
  • Obtain and maintain eligibility for insurance and Services and document in the Electronic Medical Record (EMR).
  • Effectively guide clients to community resources, assisting them in navigating systems and entitlement programs. Facilitate internal and external referrals to health and support services, ensuring comprehensive followup until resolution is achieved.
  • Maintain face to face, telephone, and other encounters with and on behalf of the client and document progress made toward achievement of goals.
  • Provide assistance in scheduling specialty care appointments and follow up as needed
  • Update the eligibility information: insurance, income, residence every six months;
  • Document progress made toward achievement of goals in the EMR. Provide support and advocacy to the client.
  • Update the action plan and eligibility information: insurance, income, residence every six months.
  • Attend multidisciplinary team meetings and provide input on retention efforts
  • Participate in health fairs, community events, provider meetings, and required trainings
  • Other duties as assigned

Qualifications:

Education:


  • Bachelor's degree in social work, psychology, counseling, human services, or a related field. Master's degree preferred.

Experience:


  • Minimum of 2 years of relevant experience in case management, care coordination, or a related field. Experience working with diverse populations is highly desirable.
  • Exceptional written and verbal communication skills to effectively interact with clients, colleagues, and external partners.
  • Demonstrated empathy and cultural competence to understand and address the diverse needs of clients. Experience working with individuals from various backgrounds is an asset.
