The Transition Counselor at the TAP Center in Sandy Hook, CT is responsible for providing essential services to transitioning Service Members and their Family Members.

This includes offering individual counseling to facilitate personal interactions, answer questions, provide clarification, and offer assistance in creating an Individual Transition Plan (ITP).

The Transition Counselor also coordinates and schedules local briefings, sets up classrooms, and announces events related to Department of Labor (DOL) Employment Workshops and VA Benefits Briefings as needed for the Soldiers.

They also conduct Transition Briefings to inform clients about available transition services and eligibility requirements for transition benefits.

Additionally, the Transition Counselor notifies eligible Soldiers about the mandatory pre-separation counseling and conducts individual counseling sessions to develop an ITP, create a resume (federal or private sector), and discuss and establish individual transition goals.

They also educate clients about the continuum of service and provide follow-up counseling to assist in completing the deliverables for the Soldier's gap analysis.

The Transition Counselor is also responsible for identifying, scheduling, and conducting TAP employment assistance workshops, as well as conducting regional outreach and pre-separation counseling.

All tasks performed by the Transition Counselor are in accordance with the instructions of the Contract Installation Manager (CIM) and in compliance with the Performance Work Statement (PWS).
