JOB SUMMARY:
The Nurse Care Coordinator is a Registered Professional Nurse who provides direct assessment, evaluation and teaching to meet the needs of individual and families within their home and communities.
QUALIFICATIONS:
- Registered Professional nurse in the state of New
- At least two years experience as a Registered or Practical Nurse with one year administrative/supervisory experience in community health
- Completion of a baccalaureate degree in nursing or an associate degree with at least 1year supervisory experience
RESPONSIBILITIES:
- Completes assessments of patient and family to determine home care needs at regular established
- Provides a complete physical assessment and history of current and previous illness(es).
- Regularly reevaluates patient nursing needs and processes information to comply with all Federal, State and City regulations, as well as all agency Policies and Procedures.
- Develops the plan of care with the patient and family; establishes goals based on nursing diagnosis and incorporates therapeutic and preventive nursing actions.
- Includes the patient and family in the planning process and makes necessary revisions as patient status and needs change.
- Provides health care instructions to the patient/family as appropriate per assessment and
- Instructs, supervises, and evaluates home health aide care provided as stipulated by law and
- Prefills medication boxes as per MD's
- Communicates with the physician regarding the patient's needs, plan of care and reports and changes in the patient's condition; obtains/receives physician's orders as required and prepares clinical
- Submits all documentation on a timely
- Identifies discharge planning needs as part of the plan of care development and implements prior to discharge of the patient.
- Investigates and reports all incidents and complaints as per agency
- Ensures that arrangements for equipment and other necessary supplies and services are
- Communicates with Director of Patient Services, agency administrator and other health related persons after new patient admission, resumption of care, and whenever the patient status changes.
- Anticipates the needs of the patient and recommends changes to Doctor and family as
- Assures that all logs are maintained as required by
- Provides coverage for any other agency site as so requested by
- Participates in oncall duties as defined by the oncall
Salary
- $95,000-110,000
Work Location
- Three days in SI Office (1415 Richmond Ave SI)
- Two days in the Field (Brooklyn and Staten Island)
