    Nurse Practitioner - PRN/Part Time

    About the Position:

    During thorough visits lasting up to an hour, Matrix providers assess a member's health, medical history, medication adherence, social environment, and other factors. This comprehensive approach offers valuable insights into a member's overall health that may be challenging to obtain in standard office visits. The Matrix Comprehensive Health Assessment enhances the quality of care provided and helps in addressing multiple care gaps in a single visit.

    Responsibilities

    Matrix Provider - What to Expect:

    • Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, physician referrals, and case management according to Matrix guidelines
    • Collaborate with healthcare professionals and caregivers as needed
    • Work in various settings such as Home Visits, Skilled Facility Visits, and Telehealth Visits

    Nurse Practitioner - PRN/Part Time - Home Risk Assessments

    Type: PRN/Part Time - Flexible-Nurse Practitioner

    PAY: $90.00 or $95.00 (based on county) per completed Assessment

    Location: Home, SNF, Telehealth and other community environments in Sandusky, Ohio

    Hours: Days, Evening, Nights, weekend, Flexible Scheduling

    Benefits: PRN/Part Time providers receive 401(k) and limited Voluntary Benefits

    Mileage Reimbursement plan


    Qualifications

    Must Haves:

    • Master's Degree or equivalent experience with NP licensure
    • Active RN and NP licensure in the state of practice
    • Board certification in a Matrix approved specialty
    • Current BLS, ACLS or CPR certification
    • Minimum 1 year of Nurse Practitioner experience
    • Proficiency in health/medical record software
    • Strong communication skills
    • Adaptability to change

    Travel Requirements:

    • Ability to travel with valid driver's license and access to a vehicle
    • Travel to patients' locations as needed
