Position:
Temporary Nutrition Services Field Coordinator


Position Responsibilities:

Work as a member of the Nutrition team to ensure compliance with the Executive Office of Elder Affairs (EOEA) requirements of the Title IIIC program as it relates to home delivered meals.

Working with AgeSpan staff, local Councils on Aging (COAs), and other community partners, the coordinator is responsible for the day-to-day referrals and changes to meal schedules.


Essential Functions:
(Core duties or tasks that are fundamental and not marginal to the performance of the job.)

  • Helps manage the daily operation of the Meals on Wheels program, supportive living sites and all AgeSpan Nutrition Programs in an assigned area.
  • Work with the Director of Nutrition and Site Coordinators to ensure proper coverage of Home Delivered Meals and Supportive Living Sites.
  • Regularly visit and monitor Meals on Wheels and Council on Aging Sites to review and direct all operations functions.
  • Handles phone calls, transfers, and documents appropriate messages with reference to Nutrition program matters.
  • Collaborates with Councils on Aging staff, information and Referral staff, Home Care CM, Geriatric Service Coordinators, and AgeSpan supervisors to coordinate referrals for and changes to service delivery of Home Delivered Meals (HDMs)
  • Communicates with Care Managers regarding starting, suspending, resuming, and terminating meals
  • Informs Care Managers and or emergency contact for National Aging Program Information System (NAPIS) consumers, of any concerns received from the meal delivery as part of the wellness check
  • Maintains MOW Driver route information in computer databases and assigns clients to specific routes. Documents all activities related to the consumer and ensures the care plan and route information is accurate and up to date.
  • When needed, produce kitchen report, create route sheets, print and/or send to COAs for the next service day
  • Assists with ordering HDM and congregate meals when needed
  • Participate in all staff meetings, training sessions and all AgeSpan agency meetings and trainings related to the position.
  • Note all Nutrition program feedback from MOW Drivers, Driver Supervisor, Lead Driver and COA Staff, external and internal clients.
  • Deliver meals to consumers when short staffed
  • Ensure all drivers and volunteers understand and follow safe food handling practices.
  • Performs miscellaneous duties and assignments as directed by Nutrition Manager

Qualifications

  • High School Diploma or equivalent
  • Valid Driver's license and personal, reliable transportation required.
  • Excellent Communication skills, bilingual in Spanish Strongly Encouraged
  • Proficient in Microsoft Office software skills
  • Strong collaboration and problemsolving skills
  • Good timemanagement and multitasking skills
  • Flexible and able to adapt to changing circumstances
  • Independent worker but also able to collaborate and be part of a team
  • Customer Service Experience

Hours: 37.5 per week;
