We're seeking people experienced in working with people with developmental disabilities to join us


FREE has an excellent opportunity for a
part-time Service Coordinator. The Service Coordinator provides targeted support to the wellness and recovery goals of individuals with complex and/or chronic behavioral health issues and needs by implementing targeted acute interventions designed to provide timely, high-quality, and efficient care.


In addition, you will:


  • Work as part of an interdisciplinary team to ensure the coordination of resources for individuals enrolled in the Outpatient Program.
  • Identify service gaps and locate needed resources for individuals, including housing, works placement and other supports.
  • Conduct routine follow up/monitoring of individuals and their services.
  • Assist individuals in understanding, managing and maximizing their resources/benefits to ensure they can receive appropriate and supportive services.
  • Provide transitional care, including appropriate followup from inpatient to other settings.
  • Provide support to people served in navigating complex health care and social services systems.
  • Provide individual and family support including advocacy when needed.
  • Promote hope and recovery by using strengthsbased, culturally appropriate, and personcentered practices.
  • Maximize community integration and normalization by ensuring community and programmatic resources are sought out and engaged upon.
  • Consult with Clinicians to devise and manage effective ongoing care plans to at risk clients.
  • Prevent, recognize and report abuse toward the people we support.
  • Perform other duties as assigned.

We require:

High School Diploma or equivalent required (Associate Degree in Human Services field preferred):

At least two years of experience working with behavioral health population
:


  • Proficient knowledge of community resources for referrals.
  • Organizational and communication skills which would enable effective interaction with fellow team members, individuals/clients/families and the community.
  • Critical thinking and sound judgment skills are necessary.
  • Computer literacy and familiarity with Microsoft Office Suite.
