We are a privately held skilled nursing facility in Verona. Benefits are available after 60 days and PTO is available after 90 days.


The Physical Therapist Assistant implements individualized therapy programs designed to restore, reinforce, and enhance performance under the supervision of a Physical Therapist.

The Physical Therapist Assistant assesses residents' physical and mental abilities and analyzes the data to determine realistic rehabilitation goals.

In addition, the Physical Therapist Assistant instructs, educates, and trains residents and caregivers in those skills and functions essential for promoting independence.


Position Responsibilities

  • Assists in the assessment, treatment, and implementation of resident care in accordance with the State Practice Act.
  • Assists residents under the direction of a Physical Therapist to maximize their potential in functional activities, mobility, activities of daily living including selfcare, meal skills, cognitive functioning, and manipulation skills.
  • Develops appropriate treatment methods in collaboration with residents and caregivers and implements the therapy treatment plan accordingly.
  • Physical therapy services include but are not limited to:
  • Therapeutic exercise includes aerobic capacity conditioning, balance coordination, agility training, body mechanics, postural stabilization, flexibility exercises, gait training, strength, power, and endurance training
  • Self-Care activities to develop or regain physical/mental functioning and/or adjust to disabilities by implementing programs involving practice in functional skills, homemaking skills, activities of daily living, and sensorimotor, educational, recreational, and social activities.
  • Cognition Treatment
  • Functional training with transfers, bed mobility, and gait.
  • Use of electrotherapeutic, physical agent and modalities
  • Meets resident care hours requirement for rehab gym, or as directed by Rehab Director.
  • Attends and contributes to resident care conferences and other related meetings.
  • Assures continuation of therapeutic plan following discharge by designing home exercise programs; instructing residents, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or home health followup programs.
  • Documents resident care services by electronic charting in resident and department records.
  • Provides therapy interventions to inpatient residents, outpatients, and Home Health clients as scheduled by Rehab Director.
  • Participates in ongoing community marketing and outreach opportunities as directed by Rehab Director.

  • Develops therapy staff and facility staff by providing information, developing, and conducting inservice training programs.
  • Complies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; and advising management on needed actions.
  • Performs other related duties as required.
