- Develops and coordinates an organized bereavement program for hospice families and residents of nursing facilities.
- Ensures that the interdisciplinary team makes an assessment of survivor risk factors at the time of the patient's admission and continually monitors. Develops an appropriate plan for follow-up services needed.
- Works with the Volunteer Coordinator to provide education and training for the volunteers that provide services for the bereavement program.
- Coordinates and facilitates weekly community bereavement support group.
- Completes current documentation of family assessments, plans, and interventions.
- Provides bereavement education for staff and the community upon request.
- Ability to be at work and on time.
- Ability to follow company policies, procedures and directives.
- Ability to interact in a positive and constructive manner.
- Ability to prioritize and multitask
- Minimum of two years recent experience in a health care setting required.
- Advanced education/experience related to grief, loss, death, bereavement and counseling required.
- Oncology or hospice experience preferred.
- Knowledge of hospice philosophy required with the ability to articulate the needs and interventions required for the patients and families.
- Master's Degree in Social Work required.
- Current California driver's license with valid California car insurance required.
- California Licensed Clinical Social Worker (LCSW) license preferred.
- 1 year bereavement counseling and support experience preferred.

Position Summary
This position requires the full understanding and active participation in fulfilling the mission of SaddlebackMedical Center.

Asa member of the Interdisciplinary Hospice Team, the Coordinator, Social Work, Bereavement is responsible for the Hospice Bereavement Program, and oversees the counseling of hospice patients, caregivers, families and bereavement services provided.
Counseling includes emotional, psychosocial and
piritual support and services provided before and after the death of the patient to assist with issues related to grief, loss and adjustment.
