    We are seeking a Care Support Nurse works primarily under the direction of the Director and/or ADON.

    Reports to: Asst Director of Nursing/Director of Operations

    Qualifications:

    • Able to read, write, understand and following directions
    • Must complete and pass all State mandated training
    • Must have computer skills for electronic medical records and documentation
    • Progressive nursing leadership experience demonstrating solid management is necessary for this nursing position.
    • Ability to carry out all responsibilities in a thorough and timely manner
    • Minimum of 24 months experience with the health needs of the population served.

    Responsibilities:

    *Oversee the day to day nursing supervisory operations

    *Scheduling of nurses as needed

    Prepares medications, administers medications, and treatments with knowledge of purpose and adverse effects of nursing care provided.

    Completes all documentation for observation, planning, and evaluation of care provided to residents, including orders by physicians.

    Functions in monitoring capacity as assigned for completion of resident's care, employee guidance, and discipline.

    Maintains open lines of communication between nursing shifts, as well as other departments, supervisors, physicians, and families of residents.

    Acts on the resident's behalf to assure patient rights and to prevent resident abuse and neglect.

    Acts on first aid knowledge in emergencies.

    Is aware of safety issues, accomplishing responsibilities in a safe manner, and reinforces good safety practices displayed by other staff members.

    Can maintain confidentiality of information.

    Works as a team Leader/Member.

    Is able to perform the following physical activities: Kneeling, Stooping, Standing, Reaching, Pulling/Pushing, Walking, Grasping, Hearing, Talking, Sitting, Repetitive Motions.

    Knowledge and Skills:

    • Effectively intercede with family and residents and resolve concerns related to the delivery of health care services.
    • Skilled in managing and interacting with a staff of different cultural and demographic backgrounds.
    • Ability to think strategically, critically and creatively.
    • Ability to interact effectively with counterparts within and outside of 5 Star Cares
    • Ability to foster and model effective customer service.
    • Ability to coach, counsel and develop employees, including those of different racial, cultural and demographic backgrounds.
    • Ability to provide clear expectations and directions.
    • Ability to solve complex problems.
    • Develop, maintain, and evaluate resident/client and staff data collection systems and processes to support the practice of nursing and the delivery of resident care.
    • Collaborate with residential staff at all levels, interdisciplinary teams, executive officers, and other stakeholders in the development, implementation, and evaluation of programs and services.
    • Develop and implement recruitment and retention programs for staff.
    • Evaluate factors related to safety, outcomes, effectiveness, cost, and social impact when developing and implementing practice innovations.
    • Delegate responsibilities appropriate to license, education, and experience of staff.
    • Utilize evidence-based practice to develop and implement standards of practice that guide practice improvement initiatives.
    • Recognize the diverse skill levels of individual staff members and the unique needs of the resident population when developing staffing plans; ensures level of staffing for the level of acuity is appropriate 24 hours/day.
    • Evaluate staff competency by direct observation, peer review, orientation and employee input, documenting this in the appropriate system.
    • Develop and implement effective educational programs designed to assist the staff in the progression from novice to expert in the care of the resident and family.

    A little bit about our company. We provide 24-hour care for the most medically fragile individuals with intellectual/developmental disabilities and have a staff to patient ratio of 1:6.     Job Type: Full-time (8-hour shifts); must be able to work 2 weekends per month and be flexible.

    Rate of pay: $28-$32 hour depending on experience
