Founded in 2017, Covenant House Illinois (CHIL) provides critical protection and resources for youth ages 18-24 experiencing homelessness throughout Chicagoland and across Illinois.

With a newly renovated base of operations, increased capacity to serve greater numbers of youth, a slate of new and expanded programs, and a recently enacted strategic vision guiding growth and service delivery over the next three years, CHIL is primed to become Chicago's premier agency serving young people experiencing homelessness.


Primary Responsibilities:


The Clinical Case Manager (CCM) provides direct physical and supportive services in a positive and nurturing manner to the young people we serve in Covenant House Illinois' Youth Development Center and Residential Program using an integrated, multidisciplinary team approach.

Reporting to CHIL's Director of Programs and Program Implementation, the CCM the Youth Development Coordinator will work with program Youth Advisors and community partners to ensure the delivery of robust, comprehensive services to youth while ensuring implementation of agency philosophy, policy, and procedures.


Qualifications:


  • Master's degree in social work or related field required.
  • 24 years of youth development experience required.
  • Licensure in Social Work, psychology, counseling or human services related field (any level) preferred.
  • Effective written, verbal, listening and communication skills
  • Ability to exercise appropriate judgment in crisis situations, with effective interpersonal skills
  • Must be willing to function as an active member of an interdisciplinary team.
  • We operate a 24/7 facility which means full time staff are expected to work some weekends and holidays.
  • Excellent organizational skills.
  • Ability to monitor and deescalate in a crisis situation.
  • Ability to follow instructions, prioritize assignments and work in a fastpaced environment.
  • Flexible work schedule to include evenings, weekends and holidays
  • Bilingual a plus
Essential duties

  • Ensure the highest quality of delivery of services to youth in accordance with the CHIL Principles, Mission and Core Values.
  • Promote a safe and healing environment by utilizing the principles and practices of traumainformed care, harm reduction and positive youth development in all aspects of work and youth engagement, including crisis prevention, intervention, and deescalation.
  • Manage a caseload of YDC and residential youth.
  • Complete a Comprehensive Assessment with each youth
  • Collaborate with a youth in developing both short term and longterm goals as part of developing and implementing an Individual Service Plan (ISP).
  • Ensure that youth have sufficient and proper ID (Birth Certificate, School Identification, State ID, and Social Security Card).
  • Meet with youth weekly
  • Ensure that intakes and progress notes are documented accurately in all Databases
  • Responsible for advocating on behalf of youth with outside service providers.
  • Attend appointments and services when needed or requested by a youth.
  • Ensure that youth's files are compliant to all grant, government agencies and local, state and federal regulations.
  • Assist with program audit and review preparation.
  • Research and keep current external referral services.
  • Work collaboratively with both internal and external service providers.
  • Develop motivational tools for youth.
  • Maintain healthy relationships with the youth, mindful of boundaries and mutual respect.
  • Model caring engagement with youth.
  • Support Youth Advisor's and floor staff whenever possible and/or needed
  • Participate in all required trainings, supervision and staff meetings.
  • Manage the day to day needs of the program i.e. processing/tracking bus cards, etc.
  • Help ensure that proper documentation and data is being captured in ETO.
  • Facilitate groups on various lifeskills issues, such as hygiene, problem solving, conflict resolution, selfesteem, and problem solving
  • Support youth advisors in providing youth engagement practices on the floor

Knowledge, skills, and abilities

  • Understand and comply with all agency policies and procedures.
  • Have an understanding of and support the implementation of Trauma Informed Care, Harm Reduction, and Positive Youth Development.
  • Have knowledge and experience working with the LGBTQIA+ population
  • Is approachable, relatable, builds rapport well, and practices active listening with all individuals.
  • Knowledge of community social service resources to assist youth with needs related to housing, employment, education, and socialemotional wellbeing.
  • Knowledge of crisis intervention and ability to address conflict between youth; able to respond quickly and safely while calmly deescalating the crisis situations and emergencies.
  • Can maintain confidentiality while understanding and complying with requirements of mandated reporting, HIPAA, and other limits to confidentiality.
