    Description

    Qualifications:

    • Graduate of an accredited school of practical nursing with two (2) years clinical experience; one year (1) in the nursing home environment and/or hospice is preferred.
    • Currently licensed as an LPN through the Ohio Board of Nursing; must be able to obtain a license in the state of Michigan.
    • Verification of successful completion of a medication course.
    • Has working experience charting in an EMR system, Net Smart system preferred.
    • Has the ability to work in collaboration with the interdisciplinary team.
    • Possesses strong verbal and written communication skills; demonstrates ability to communicate effectively with patients, families, and team members regarding care issues.
    • Demonstrates strong organizational, time management, and prioritization skills.
    • Must be a licensed driver with an automobile in good working condition and is insured in accordance with state and agency requirements.
    • Must be CPR certified according to the American Heart Association CPR standards.
    • Understands and is committed to the hospice philosophy.
    Essential Job Responsibilities


    • Performs direct nursing care, nursing interventions and prescribed treatments based on established plan of care (POC) and hospice policy and procedures as delegated by the RN.
    • Observes and reports patient's physical, emotional and spiritual needs as well as changes in the patient's condition and response to treatment plan to the RN.
    • Administers medications according to physician orders and hospice polices and procedures. Obtains and processes physician orders.
    • Provides appropriate education and information to patients, families, and nursing home facility staff (if applicable) related to the patient's plan of care and needs.
    • Completes clinical documentation according to agency standards.
    • Responsible for learning and utilizing technology made available by the agency to increase efficiency, communication and performance of duties.
    • Participates in the agency's Quality Improvement program.
    • Attends staff meetings and completes all mandatory training.
    • Holds in strictest confidence all patient information and discloses information and data only to persons authorized by Hospice.
    LPNs may have exposure to infectious diseases.

    This position will require driving to patient homes and facilities in various weather conditions and will be subject to day and/or night-time travel.
