Description
Forever Young Adult Day Health Care Center is currently looking for a social work assistant.


Qualifications:


  • Fluent in English/Spanish.
  • BA/BS in Social work, sociology, gerontology, or psychology ONLY*.
  • Experience working with the elderly 1+
  • Social service experience a plus.

Duties (include but are not limited to):

  • Conduct initial assessment and reassessment under the supervision of a MSW.
  • Document flow sheet, progress notes & quarterly notes.
  • Help arrange & schedule transportation
  • Other duties as assigned by Program Director

Job Types:
Full-time, Part-time


Pay:
$ $20.00 per hour

Expected hours: 30 per week
