    Description

    PURPOSE AND SCOPE: Offers nutritional services for in-center and home patients to enhance their nutritional status and boost clinical outcomes. Works within the healthcare team to support Quality Enhancement Program (QEP) and Quality Assessment and Performance Improvement (QAI) initiatives.

    PRINCIPAL DUTIES AND RESPONSIBILITIES:

    • Educates patients on diet and kidney disease, customizing the information to their learning needs.
    • Provides detailed nutritional education to patients and families, covering various aspects such as nutritional status, lab results, and fluid levels.
    • Participates in comprehensive assessments and assists in developing individualized care plans.
    • Determines diet prescriptions based on best practices.
    • Documents patient progress notes accurately and adheres to guidelines.
    • Offers ongoing nutrition assessment and counseling to sustain effective nutritional status.
    • Collaborates with the team on treatment recommendations and adjustments.
    • Reports on nutrition quality improvement activities.
    • Engages in Pre-ESRD education and coordinates care plans with Case Managers.
    • Reviews and acts upon nutrition-related lab results.
    • Attends staff meetings and maintains competence in practice.
    • Ensures safe and clean work areas.
    • Performs assigned nutrition management tasks.
    • PHYSICAL DEMANDS AND WORKING CONDITIONS:
    • The role involves desk work, computer interaction, and patient/facility staff engagement.
    • Requires travel between facilities and community locations.
    • EDUCATION AND REQUIRED CREDENTIALS:
    • Must be a Registered Dietitian with state licensure and relevant certifications.
    • EXPERIENCE AND SKILLS:
    • Minimum of 1 year in clinical nutrition, leadership skills, strong communication, and organizational abilities are essential.
    • Basic computer proficiency is required.
