POSITION TITLE (Full-Time)
Living Room Community Support Specialist


QUALIFICATIONS

Required:
Master's Degree in field of Behavioral Science and documented internship working with people diagnosed with mental illness or obtaining Master's degree in the next 3 months


REPORTS TO
Living Room Supervisor

Director of Mental Health


GENERAL RESPONSIBILITIES

  • Provide behavioral health care services and supports to guests and collateral contacts. Works both at Leyden Living Room (LLR) and on an outreach basis in the local community.
  • Perform other related duties as assigned by the LLR Supervisor and/or Director of Mental Health.
  • Adhere to Leyden administrative policies and federal and state regulations, including Illinois Rule 50.
  • Living Room Hours are 24/7. This shift seeks employees for the 9am9pm shift. Any employee training will be conducted during the daytime.

SPECIFIC DUTIES

  • Design and implement policies and procedures, in conjunction with the Living Room Supervisor and Director of Mental Health.
  • Work as a part of the LLR to provide crisis intervention and stabilization, community support, and case management services onsite, which includes day/evening, rotating weekends, and rotating holidays.
  • Participate and assist in the daily operations and activities of the Living Room
  • Provide reports to LLR as requested
  • Evaluate and screen individuals to help determine needed services and support
  • Provide all services and supports with sensitivity to age and cultural diversity
  • Provide case management services, including but not limited to: housing, entitlements, access to healthcare and assisting in the development of guest independent living skills
  • Participate in the design and implementation of programs and activities as needed.
  • Be available for afterhours consultation and crisis response
  • Participate in crisis intervention as needed and response within a reasonable amount of time depending on the nature of the crisis. Examples include but are not limited to psychiatric/medical hospitalization, safety issues, etc.
  • Participate in team staff meetings
  • Participate in the development of treatment interventions
  • Coordinate treatment with other agency program staff
  • Complete all Agency and state agency records and forms according to specific guidelines
  • Assist in identification and achievement of unit goals
  • Assume responsibility for continued professional growth
  • Represent the Agency to community groups and service providers
  • Maintain and facilitate relationships with community agencies and resources
  • Interpret Agency services to community groups and service providers

SPECIAL REQUIREMENTS AND PHYSICAL DEMANDS:


  • Requires safely lifting or moving up to 25 lbs. on an occasional basis, for example movement of materials, files or texts.
  • Regular requirement to stand, and sit for long periods, with some walking, occasional bending, stooping, squatting, pushing and pulling.
  • Noise level is usually quiet to moderate.
  • Master's Degree Required
