Clayton Residential Home is a Specialized Mental Health Rehabilitation Facility in Chicago's Lincoln Park neighborhood. Clayton is a seeking a full-time Clinical Case Manager for a
Sunday through Thursday 2:30pm - 11:00pm shift. We provide Recovery-oriented, person-centered mental health services to adults living with mental illness.

Our facility provides holistic services with a staff consisting of social workers, mental health professionals, nurses, nursing assistants, and direct-care staff.

We offer individual and group programming, a variety of activities and therapeutic services and 24-hour supervision. Our residents are working on gaining and maintaining mental health stability and developing independence. Many will move out of Central Plaza and into an apartment with community support.


Our Clinical Case Management team provides mental health counseling and supportive services in partnership with the residents to help them work towards achieving their individual goals.

The Clinical Case Manager partners with a caseload of resident to help them treatment plan and achieve their goals they set for themselves.

The Clinical Case Manager provides counseling interventions, crisis management, treatment planning and programming scheduling to help the residents work towards their stated goals of independence.

This person works collaboratively with their team members on the social service team, their supervisor and other department staff to ensure that services provided to the residents are recovery oriented.

A Bachelors and/or Master's Degree in Social Work or Counseling or a related human services field is required. At least one year of experience working in a mental health setting is preferred.

This is an evening position with a schedule of Sunday through Thursday 2:30pm - 11:00pm.

From $23.10 per hour
