    Wound Care Nurse

    $115,000 - $120,000


    A well-established skilled nursing facility in Brooklyn is seeking an experienced Wound Care Nurse to oversee and manage wound care services throughout the facility.

    This is a specialized, clinical-focused role for a nurse who is confident working independently and takes ownership of wound care outcomes.

    The Wound Care Nurse will be responsible for comprehensive wound assessments, treatment planning, ongoing monitoring, and ensuring that all wound care protocols are followed accurately.

    This role works closely with nursing leadership, physicians, and therapy teams to ensure optimal healing outcomes and regulatory compliance.

    Key Responsibilities:

    • Perform detailed wound assessments and develop individualized treatment plans
    • Monitor wound healing progress and modify care plans as clinically appropriate
    • Provide hands-on wound care treatments, including complex and chronic wounds
    • Ensure accurate, timely documentation in accordance with facility policies and DOH regulations
    • Collaborate with nursing staff, medical providers, and rehabilitation teams
    • Educate nursing staff on wound care best practices and prevention strategies
    • Participate in audits, surveys, and quality improvement initiatives related to wound care
    • Maintain compliance with infection control and skin integrity protocols

    Requirements:

    • Active New York State RN or LPN license
    • Proven wound care experience in a skilled nursing, long-term care, or rehabilitation setting
    • Strong clinical judgment and ability to work independently
    • Excellent documentation, organizational, and communication skills
    • Wound care certification is a plus, but not required
