Summit Home Health Care is seeking a full time
Home Care Community Liaison / Marketing Specialist.

Salary $60,000 to $95,000 per Year depending on Experience, Plus a productivity _Bonus Structure_**Job Responsibilities of the
Home Care Community Liaison / Marketing Specialist:

Bring workable patient referrals to the agency
Develop new referral sources in the community
Strengthen relationships with new and existing referral sources daily visits to organizations, physician offices, hospitals, assisted livings facilities and adult day care to educate and inform referral sources and patients of home health options
Preplan weekly marketing activities and followthrough
Event coordination and community liaison participate in health fairs, awareness days, etc.

Job Qualifications of the
Home Care Community Liaison / Marketing Specialist:

Prior experience in Home Health Care is recommended, but we will train the right individual
Knowledge and understanding of Medicaid/Medicare guidelines and MLTCs is a plus
Excellent customer service, attention to detail and communication skills a must
