Description
Greater Portland Health is expanding its Community Health Worker team and seeks a dynamic individual to join us.

This individual will facilitate access and improve the quality of health care for our patients through a range of activities such as outreach, community education, informal counseling, social support, and advocacy.


Employment Requirements

  • High School Diploma or GED Certificate required for all positions
  • Level 3 English required
  • BLS required or within 6 months of hire
