Job Summary:


Reporting to the Clinical Supervisor, the Community Health Worker will primarily be working out in the community with specific target populations throughout our Bronx & Manhattan, NY territory.

CHWs will work closely with Care Coordination RNs and LSWs to improve patient care and outcomes.


Duties & Responsibilities:


  • Responsible for establishing rapport with clients and their caregivers
  • Follow up with clients via phone calls and home visits
  • Help clients set personal health related goals
  • Provide referrals for services to community agencies as appropriate
  • Help clients connect with transportation resources and provide appointment reminders when needed
  • Work collaboratively with other Help at Home personnel assigned to the same client
  • Act as a client advocate and liaison between the caregiver, family, and community service agencies
  • Document accordingly in the EMR and other software no later than 24 hours after client contact
  • Attend regular staff meetings, trainings and other meetings, as requested
  • Manage assigned caseload of clients

Preferred Skills & Abilities:


  • Ability to work independently and as part of a broader care team as the situation requires
  • Excellent interpersonal, written, and verbal communication skills
  • Organized, flexible and ability to multitask

Education & Experience:


  • Bachelor's Degree preferred (not required)
  • Care Coordination/Case Management experience preferred
  • One to two (12) years of experience directly related to the duties and responsibilities specified
  • Strong preference for experience with elder population in a home health care, hospice/palliative or facility setting
