Responsibilities and Duties

  • Responsible for training and development of care management team
  • Adhere to all Endeavor policies, procedures, and state and federal regulations.
  • Onboard new clients, including conducting intake visits to assess potential client needs, help them understand the benefits of Endeavor's services, and secure them as clients.
  • Coordinate client care, including creating and managing tailored care plans, conducting periodic client calls and visits, introducing clients to new caregivers, and serving as the ongoing point of contact for clients and their families.
  • Assist in the staffing and scheduling of open shifts.
  • Participate in marketing efforts.
  • Responsible for longrange planning, fiscal viability, and quality of care provided by the branch
  • Assist other disciplines in coordinating activities when necessary, assuming responsibility for continuity, appropriateness, and quality of services delivered
  • Responsibilities Essential Functions: Develops, plans, implements, analyzes and organizes operations for the Branch
  • Responsible for the delivery of care for all clients served by the Branch by providing supervision and support to the Care Manager(s)
  • Maintains office operations in an efficient, productive, effective, and organized manner, Conducts continuous quality improvement quarterly committee meetings, reviews all patient satisfaction data, and follows up on negative patient satisfaction surveys and followup visits with referral sources

Qualifications and Skills

  • CNA License with hands on caregiving experience
  • Ability to coach and mentor Caregivers and Care Managers
  • Supervisory experience
