Job DescriptionPOSITION SUMMARY:
Responsible for developing, implementing, and managing a team-based approach tribal clinical and community-based nutrition program.
ESSENTIAL FUNCTIONS:
1. Provides nutritional counseling to individuals with complex health needs based on assessment of unique dietary goals and health conditions;
2. Provides professional guidance to help clients make healthier food choices in office or home-setting;
3. Provides follow-up to referring physicians regarding patient's nutritional status, nutrient needs, or specific diet plan;
4. Plans, develops, and implements dietary plans to address health concerns (e.g. diabetes, obesity, heart disease);
5. Document encounters in the patient Electronic Health Record (EHR) software to meet standards for billing on a timely basis (e.g., ADA, CMS);
6. Collaborates with a team of healthcare professionals, tribal departments, and/or participates in other tribal community programs to create holistic approaches to health, nutrition, and guidance related to nutrition/food safety, sanitation, and quality;
7. Conducts regular health screenings and participates in local community assessments for monitoring and evaluation of key health indicators to provide early intervention and guidance to individuals at risk for health issues related to diet and nutrition;
8. Utilizes screenings and local assessments to plan for community education, skill-building workshops, and events targeting the life span;
9. Collects and documents data as necessary for reports or grant requirements;
10. Participates and presents at community events such as health fairs, community outreach events, and other special events as needed;
11. Practices cultural sensitivity by respecting cultural nuances and incorporation of traditional foods and cooking methods into nutrition recommendations whenever possible;
12. Other duties as assigned.
MINIMUM REQUIREMENTS:
Bachelor's Degree in Dietetics, Nutrition, or related field; credentialed as a Registered Dietitian (RD) and licensed in the State of Texas; Minimum of three (3) years' work experience in clinical and public health nutrition; Must possess a valid Texas Driver's License and be insurable; Must be able to successfully pass a post-offer drug screen and criminal history background check.
KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledgeable of nutrition and medical nutrition therapy principles and concepts; Knowledge of human behavior and methods for effecting behavior change; Knowledge of population trends and community assessment; Knowledge of public health nutrition principles; Ability to communicate effectively; Ability to work independently and as part of a team-based approach; Must have good organizational skills; Must have good time management skills; Must be familiar with nutrition related ICD-10, CPT, and Healthcare Common Procedure Codes; Must have Microsoft Office applications skills; Must be able to communicate effectively in both written and verbal form; Must have good analytical skills; Ability to maintain confidential client information; Ability to obtain BLS/CPR and Food Handler's permit; Must be able to maintain patient confidentiality; Must be culturally sensitive to minority (especially Native American) populations.
PHYSICAL REQUIREMENTS:
While performing the essential functions of this position, the employee is frequently required to sit, talk, and hear. The employee is occasionally required to reach with hands and arms. The employee is occasionally required to lift up to 10 pounds. The vision requirements include close vision, peripheral vision and ability to adjust focus.
