Overview:

The Crisis Peer Support Coach assists individuals throughout recovery, helping them gain skills, access resources, and regain the confidence to succeed in society.

They will initiate contact with patients in crisis and support them throughout the recovery process by providing guidance, advocation, validation, respect, and empowerment drawing on their lived experiences.

This position works in acute settings that address crisis mental health and substance use care between crisis settings as well as in an outreach setting with a multidisciplinary team.


Responsibilities:


Provide Peer Counseling and Follow-up Care

  • Work as a member of an interdisciplinary Mobile Response Team with law enforcement, providing outreach services to people in crisis.
  • Provide direct peer counseling based on your own lived experiences.
  • Share personal recovery stories and describes personal experiences to inspire and support individuals throughout the recovery process.
  • Work closely with patients to engage them in the decisionmaking process and help them understand the benefits of assessment and accessing the appropriate level of care as indicated and employ recovery strategies.
  • Develop appropriate intervention and recovery plans for individuals, families, and groups.
  • Model and support health and wellness activities, and healthy coping skills and boundaries for patients.
  • Advocate and assist patient navigate for their basic needs, including transportation, shelter, food, etc.
  • Act as an advocate for individuals to facilitate positive changes in their recovery.
  • Monitor and participate in outreach to patients to ensure adherence to treatment recommendations and address barriers to adherence.
  • Meeting with patients regularly to offer oneonone support and counseling.
  • Actively participate in continuous outreach to clinic patients via telephone and assists in scheduling clinic and dental appointments.
  • Requires working in a 24x7 model and must be available to work night and weekend shifts.
  • May be crosstrained/scheduled to work in the Clinic Health Center setting.
  • Acts in a manner that protects the integrity of the Health Care District's Mission.
  • Emergency duty may be required of the incumbent including working in Red Cross shelters or performing other emergency duties including, but not limited to, responses to threats or disasters, manmade or natural. Additionally, incumbents are required to perform emergency response and management duties for Palm Beach County as required.

Serve on Interdisciplinary Team, Collaborate to Ensure All Patients Receive Appropriate Care

  • Work with a multidisciplinary psychiatric and substance use team mixed with clinical and nonclinical team members collaboratively to provide excellent patientcentered care.
  • Consult with supervisor and/or psychiatrist and medical team as needed.

Collaborate with Appropriate Systems Involved with Patient Care

  • Work collaboratively with communitybased partners regarding incoming and outgoing referrals to provide patientcentered coordinated care.
  • Knowledgeable of various community organizations and resources to provide additional support and services.

Complete all Administrative Tasks/ Functions Appropriate to the Role to Support Operations

  • Show competence in utilizing the EHR and other IT solutions regarding the patient's episode of care.
  • Responsible for registering patients in electronic health records (EHR) and completing all documentation and required data fields.
  • Maintain confidentiality of EHR records relating to patients' treatment as per HIPPA laws.
  • Participate in all assigned meetings, staff development, and training as required.
  • Remain in compliance with internal and external policies, procedures, regulations, and requirements, State Licensing standards, County Monitoring requirements, and managed care regulations.
  • Assists other employees by relaying instructions, coordinating efficient workflow, and requesting information as needed.
  • Utilize various software systems to identify patient needs and implement interventions regarding connecting them to other community partners who provide treatment care or services for social determinants of health.

Qualifications:

Education:


  • High school diploma or G.E.D. required.
  • Lived experiences as a person engaged with mental health services or having survived severe addiction is required; previous experience or training as a Recovery Peer is desired.

Experience:


  • A passion for assisting those with mental health or substance use challenges. Preferably, applicant has lived experience with mental health or substance use struggles personally or through a family, member, friend, or collegue.

Certification:


  • Recovery Peer Specialist Certification or the achievement of this certification within 1 year of employment.
  • Current ACLS or BLS and CPR certification/training required.

Licensure:


  • Valid Florida driver's license requi
