Our Patient Care Associates (PCA) collaborate with a multidisciplinary team to develop and carry out a patient centered plan of care.

This position involves treating a diverse patient population with a variety of medical conditions and post surgical care needs.

The medical surgical PCA provides direct nursing care in accordance with Bristol Health's established policies, procedures, and evidence based protocols.


Tasks and responsibilites include:
Performs a wide variety of patient care tasks utilizing a Patient Focused Care model.

Follows standardized procedures prescribed in a clinical pathway, including activities related to direct care, diagnostic testing, patient transport and documentation, as directed by RN.

Orienting and mentoring new team members, following universal precautions utilizing appropriate personal protective equipment.


Essential Job Functions and Responsibilities:


Provides direct care to patients under direction of an RN, based on current policies and procedures, accepted standards of care, and established competencies
Assist with ADLs

Assists with elimination, records output.

Provides nourishments to patients as directed by the RN, assists with meals, records input.

Observes for and reports changes in patient condition to the RN.

Answers call lights promptly and relays information to the appropriate staff.

Obtains and records vital signs as directed.

Records accurately and appropriately all I & O interventions.

Able to prioritize patient care and unit activities.

Weighs patients and documents according to policy.

Independently performs EKGs according to policy.

Independently performs phlebotomy as directed according to policy.

Is aware of limitations of patient care associate and functions within those limits.

Provides for the patient/family needs for psychological, emotional or spiritual support in an unhurried, empathetic manner.

Assumes role of patient advocate.

Consistently refers to the critical pathway when rendering care.

Responds to requests for colleagues in an acceptable time period.

Transports patients safely throughout the facility.

Documents patient care activities, safety measures, actions, and/or communications with patient/family clearly, concisely, timely, and accurately.

Works with colleagues and other disciplines in a spirit of teamwork, professionalism and goal attainment.

Reviews assignment with RN at the beginning of each shift to establish priorities and goals.

Reports changes in patient condition to RN in a timely manner. Collaborates effectively with other team members to meet family/patient needs.


Must be willing to be floated or reassigned to another department, unit, office, or location as needed.

Qualifications:


Educational / Minimum Requirements:


  • Basic life Support (BLS)
  • Active CPR certificate required
  • High School Diploma or equivalent
  • Active CNA certificate or One (1) year of experience in a role as a PCA, Ed Tech or CNA in acute or long term care.
  • Good Communication Skills
OR

  • 3rd year BSN student who has completed two clinical rotations
  • Medical Assistant
  • EMT

Bristol Hospital Mandated Educational Requirements:

General orientation at time of hire. Accucheck and Fire/Safety/Infection Control annually. CPR every 2 years. Additional cross-training as appropriate to perform diverse job duties. Successfully complete all training programs as determined by the Hospital. Other unit based competencies/skills as required.


Work Environment:

Normal patient care environment. Exposure to patient fluids and waste. Rapid pace and variable environment.


Cognitive Requirements:

Mental flexibility to perform diverse duties involved in patient care. Good communication skills, written and oral, and ability to follow written and oral instruction.
