Job Summary:


The Patient Care Coordinator is a healthcare professional who serves as a liaison between patients and healthcare providers, ensuring that patients receive the care they need in a timely and efficient manner.


What You'll Do:


  • Coordinates and organizes appointments and documentation to facilitate the smooth running of the healthcare environment.
  • Communicates with patients and their families to explain procedures, treatments, and care plans.
  • Collaborates with healthcare providers and multidisciplinary teams to ensure patients' medical, physical, and emotional needs are met.
  • Assists patients in understanding their health conditions and treatment options.
  • Advocates for patients' rights and needs.
  • Maintains patient records and ensure confidentiality in accordance with HIPAA regulations.
  • Tracks and report on patient care outcomes and identifies opportunities for improvement.
  • Assists in the discharge planning process to ensure a smooth transition of care.
  • Operates with the highest level of conduct, integrity, and confidentiality.
  • Complies with local, state, and federal regulations, company policies and best practices.
  • Reports any issues or incidents promptly.
  • Participates in quality improvement activities in the healthcare setting.
  • Performs other duties as assigned.

What You'll Need:


  • Excellent written and verbal communication skills.
  • Proficiency in electronic health records and clinical software systems.
  • Familiarity with electronic medical record (EMR) systems and other healthcare software.
  • Knowledge of HIPAA and other relevant healthcare regulations.
  • Outstanding problemsolving and organizational abilities.
  • Accuracy and attention to detail.
  • Ability to maintain confidentiality of conversations, data records, files, and other vital information.
  • Strong interpersonal skills, with the ability to establish rapport and maintain effective relationships with clients.
  • Ability to interact effectively with patients, families, and healthcare teams.
  • Strong clinical skills and knowledge.
  • Commitment to continuous learning and professional development.

Education and Experience:


  • High school diploma or equivalent required.
  • Bachelor's degree in healthcare administration, nursing, or a related field preferred.
  • Minimum two years of experience in a healthcare setting, such as a hospital, clinic, or doctor's office.

Physical Requirements:


  • Prolonged periods sitting, standing, bending, reaching.
  • Must be able to lift up to 20 pounds at times.
