    Nurse Practitioner – PRN/Part Time

    About the Position:

    During visits lasting up to an hour, Matrix providers evaluate a member's current health, medical history, medication adherence, social surroundings, and other risks. This offers valuable insight into a member's overall health that can be hard to gather during typical office visits. The Matrix Comprehensive Health Assessment enhances care quality and enables us to address multiple care gaps in one visit.

    Qualifications:

    • Master's Degree required OR equivalent experience and completion of NP licensure
    • Current RN and NP licensure in the state of practice, including prescription authority or eligibility to obtain such authority
    • Board certification by the AANP, ANCC, or the AACCN in a Matrix-approved specialty
    • Current BLS, ACLS, or CPR certification
    • Minimum of 1 year experience as a Nurse Practitioner
    • Strong computer skills and familiarity with health/medical record software
    • Excellent verbal and written communication skills with patients, clients, and colleagues
    • Comfortable and adaptable to frequent changes

    Travel Requirements:

    • Ability to travel, possess a valid state driver's license, and the capability to drive, with proof of adequate automobile insurance
    • Travel needed to visit patients at their residences

    Responsibilities:

    • Perform Adult/Geriatric Assessments, medical evaluations, health education, physician referrals, case management, and documentation adhering to Matrix guidelines
    • Collaborate with healthcare professionals, family members, caregivers, and ancillary medical staff as necessary
    • Coordinate with Primary Care Physician (PCP) on patient education and follow-up
    • Deliver services in diverse settings including Home Visits, Skilled Facility Visits, and Telehealth Visits

    Nurse Practitioner - PRN/Part Time - Home Risk Assessments

    Type: PRN/Part Time - Flexible-Nurse Practitioner

    PAY: $105.00 per completed Assessment

    Location: Home, SNF, Telehealth, and other community environments in Medford, Massachusetts

    Hours: Days, Evenings, Nights, Weekends, Flexible Scheduling

    Benefits: PRN/Part Time providers receive limited benefits, including 401(k) and voluntary benefits
