Clinical Pharmacist - Baltimore, United States - Greater Baltimore Medical Center (GBMC)
Description
Under limited supervision, the Transition of Care clinical pharmacist will work with an identified patient population to assure medication adherence and appropriate utilization of medications. Works within a multidisciplinary team and does their part to assist these patients achieve their optimal health status. Aligns services to better assist recently discharged patients with successfully transitioning from GBMC back to their home, ambulatory clinic or other facilities.
Education:
Pharm D from an accredited college or university or BS in Pharmacy from an accredited pharmacy school, college or university
Completion of an ASHP accredited pharmacy practice residency, specialty residency or a fellowship; or five years relative clinical experience required. Completion of a PGY2 Residency is preferred
Licensures/Certifications:
Licensure as a Pharmacist by the State of Maryland
Certification from the Board of Pharmacy Specialties is preferred
Experience:
Experience in inpatient and ambulatory clinical pharmacy practice
Board Certified in Pharmacotherapy (BCPS), Ambulatory Care (BCACP), but not required
Physical Requirements
Ability to travel different locations, includes ability to drive, negotiate stairs and walkways
Working conditions

Skills:
* Demonstrated ability to collaborate clinical care ideas across multiple patient care services and to communicate effectively as a patient advocate
* Demonstrated commitment to adhering to a high standard of personal and professional conduct and possess excellent interpersonal and communication skills and problem-solving skills
* Demonstrated ability to interact with a diverse population and work effectively with a multidisciplinary team, handling multiple responsibilities in a fast-paced environment.
Principal Duties and Responsibilities:
* Develop a transition of care program (focusing on the transition from hospital discharge to outpatient primary care follow-up) that provides medication reconciliation, medication assessments/recommendations, and medication education to identified patients
* Under a collaborative agreement, provide ambulatory clinical pharmacy services including, but not limited to, disease management and comprehensive medication review for identified high-risk patients transitioning from inpatient to outpatient
* Design and implement workflows and EMR functionality that supports patient identification, patient referral, patient scheduling, patient assessment and documentation; review patient claims and electronic medical record data to tailor patient & provider interventions
* Participate in patient care in the inpatient and outpatient settings; assess and make recommendations to optimize individual's medication therapy, and provide personalized education and information, through virtual and telephonic interactions with patients and their providers, as relevant
* Provide direct and indirect patient services as part of a broader program focused on empowering patients through self-management principles; use motivational interviewing techniques to promote self-management principles and empower patients to achieve optimal medication-related outcomes in conjunction with their health care providers
* Develop patient and provider educational interventions for medication - related problems and create materials to ensure providers have the most current drug information.
* Provide regulatory assistance with medication management policy and procedures
* Design, implement and monitor patient-specific pharmacotherapy; promote safe, evidence-based, and cost-effective use of drug therapy; review medical records and consult with patients to improve their health outcomes through optimized pharmaceutical care.
* Develop and implement clinical programs for high-risk patients
* Develop and implement patient outcome tracking reports and monitor and present data to internal stakeholder groups; present and provide clinical program information and outcomes to management team
* Assist in the development and implementation of quality improvement processes to monitor the effectiveness and quality of pharmacists' services; evaluate clinical program results and adjust programs accordingly
* Collaborate with internal stakeholder groups including but not limited to Health Partners, Inpatient Pharmacy, and the Population Health Department
* Attend meetings, participate in Committees
* Provide educational in-services for fellow pharmacists and clinic team members, mentor students, and conduct regular education sessions/grand rounds for medical community on TOC topics
* Regularly evaluate evidence based guidelines and implement the recommendations into daily practice
