Home & Health Care Management is the oldest home health care agency in Northern California.

Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties.

We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients.

Responsibilities:


This is a new position developed to review of medical records of a special MediCal Waiver program, the Home & Community Based Alternative Waiver (HCBA) program, which provides nursing and social work.


The role manages a team of people responsible for coordinating HCBA services, including implementing, monitoring, and providing quality assurance of services within the HCBA guidelines and standards.

The role reports directly to the CEO of the company.


Duties:

To perform this job you must be able to perform the following duties successfully:

  • Review medical records for all clients to ensure that care can be delivered and program guidelines are adhered to
  • Manage a dedicated clerical team and coordinate with clinical managers to meet the care needs of our clients in a timely fashion
  • Perform quality assurance audits on medical records
  • Ensure Plans of Treatment for clients are tracked
  • Ensure proper documentation, including signatures of medical providers, signatures and consents of clients and caregivers, and uploaded into an electronic medical records system.
  • Answer questions from the public regarding the program
  • Adjudicate TARS as they arise

Required Skills and Abilities:


  • Successfully managed programs and teams in previous jobs
  • Ability to work with others, at all levels within the organization, and collaborate effectively.
  • Aboveaverage and professional interpersonal, problemsolving, and written and oral communication skills
  • Able to interpret and develop administrative procedures for programs

Essential Requirements:


  • Valid California Driver's License
  • Eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • COVID vaccinated or have an exemption.

Preferred Skills, Knowledge and Certifications

  • Familiarity with medical terminology
  • Background in medical records, or related clinical experience
  • Certified RHIT, RHIA or CPHQ

Service Region/ Location:

This role is located in our Chico office. This is not a remote role and you are required to work in our office.


Schedule:

Monday through Friday from 8am until 5pm, with 1 hour for lunch. No weekend hours.


Travel:

No travel required, except for the possibility of training in another location.


Physical Requirements:


Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity.

Physical requirement for this role specifically include

  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours
  • Able to lift to 35 pounds without assistance

Compensation:

Salary is $72,800 to $80,600 per annum. Exact compensation may vary based on skills and experience.

After 60 days of employment you will be eligible for health insurance, dental insurance, vision insurance, life insurance, as well as Flexible Spending Account (FSA).


Leave accrues starting on your first day of employment, and you can take paid vacation, sick and holiday leave after 90 days.

We offer a 401(k) matching up to 4% after 1 year of employment and 1,000 hours worked.

This is an exempt role.
