    Description


    The Clinical Pharmacy Specialist (CPS) for Anticoagulation is responsible for providing and coordinating pharmaceutical care services and collaborative drug therapy management for patients referred to the Providence Health System's Anticoagulation Clinic.

    In addition, the CPS will serve as a resource to the pharmacy, nursing and medical staffs within the field of anticoagulation.

    Required Qualifications:

    • Graduate of an accredited college of pharmacy
    • Upon hire:
    Oregon Pharmacist License


    • Within 2 years of hire National Certification from Board of Pharmacy Specialties\

    Preferred Qualifications:

    • PharmD Or
    • Master's Degree With residency Or equivalent educ/experience
    • Experience in the area of anticoagulation dosing and monitoring
    • 2 years experience in hospital or ambulatory care pharmacy
