    Transition of Care (TOC) Coordinator - Full-time (40 hours)

    Monday - Friday 8am - 5pm


    Aiken Regional Medical Centers, located in Aiken, South Carolina, is a 273-bed acute care facility providing quality healthcare to the residents of Aiken and surrounding communities.

    Aiken Regional Medical Centers has been ranked a top hospital in South Carolina by the Carolina Center for Medical Excellence for its treatment of heart attack, heart failure and pneumonia.

    The Transition of Care (TOC) Coordinator is a Registered Nurse, Case Manager who is a healthcare professional that is responsible for coordinating and managing the discharge process for patients in an acute hospital setting.

    Their primary role is to ensure a smooth transition from the hospital to home or the next level of care, optimizing resources for the patient's recovery and successful healthy living environment.


    Key Responsibilities:

    • Assess patient's condition and needs to develop an effective discharge plan
    • Ensure smooth transitions between different levels of care, such as from the emergency room to the medical unit or from the hospital to home or a rehabilitation facility.
    • Collaborate with physicians, nurses, and other members of the interdisciplinary team to implement the discharge plan.
    • Coordinate and monitor the progress of the patient's care plan, working closely with the clinical team.
    • Communicate and explain the discharge plan to patients and their families, addressing any concerns or questions they may have.
    • Arrange necessary aftercare services such as home care, physical therapy, medical equipment, or follow-up appointments.
    • Work closely with insurance providers to understand patient's coverage and benefits, and to ensure the necessary services are covered.
    • Advocate for the patient's needs and preferences, ensuring their voice is heard in decisions about their care.
    • Maintain detailed records of patient interactions and discharge plans, ensuring compliance with all relevant regulations and confidentiality requirements. Including data collection for hospital and state programs.
    • Provide education and resources to patients and their families to promote health and prevent readmission.

    Qualifications


    • BSN preferred, graduation from an accredited nursing program
    • Current licensure as a Registered Nurse
    • Minimum of 3 years in a hospital setting, preferably in a discharge planning or case management
    • Excellent communication and interpersonal skills
    • Experience in facilitating change through interdiscilpnary collaboration.
    • Ability to handle high-stress situations and make critical decisions quickly
    • Strong organizational skills and attention to detail
    • Recent working knowledge of Interqual criteria and Milliman Care Guidelines preferred.
