- Assess patient's condition and needs to develop an effective discharge plan
- Ensure smooth transitions between different levels of care, such as from the emergency room to the medical unit or from the hospital to home or a rehabilitation facility.
- Collaborate with physicians, nurses, and other members of the interdisciplinary team to implement the discharge plan.
- Coordinate and monitor the progress of the patient's care plan, working closely with the clinical team.
- Communicate and explain the discharge plan to patients and their families, addressing any concerns or questions they may have.
- Arrange necessary aftercare services such as home care, physical therapy, medical equipment, or follow-up appointments.
- Work closely with insurance providers to understand patient's coverage and benefits, and to ensure the necessary services are covered.
- Advocate for the patient's needs and preferences, ensuring their voice is heard in decisions about their care.
- Maintain detailed records of patient interactions and discharge plans, ensuring compliance with all relevant regulations and confidentiality requirements. Including data collection for hospital and state programs.
- Provide education and resources to patients and their families to promote health and prevent readmission.

The Transition of Care (TOC) Coordinator is a Registered Nurse, Case Manager who is a healthcare professional that is responsible for coordinating and managing the discharge process for patients in an acute hospital setting.
