    Responsibilities

    Transition of Care (TOC) Coordinator - Full-time (40 hours)

    Monday - Friday 8am - 5pm

    The Transition of Care (TOC) Coordinator is a Registered Nurse, Case Manager who is a healthcare professional that is responsible for coordinating and managing the discharge process for patients in an acute hospital setting. Their primary role is to ensure a smooth transition from the hospital to home or the next level of care, optimizing resources for the patient's recovery and successful healthy living environment.

    Key Responsibilities:

    Assess patient's condition and needs to develop an effective discharge plan

    Ensure smooth transitions between different levels of care, such as from the emergency room to the medical unit or from the hospital to home or a rehabilitation facility.

    Collaborate with physicians, nurses, and other members of the interdisciplinary team to implement the discharge plan.

    Coordinate and monitor the progress of the patient's care plan, working closely with the clinical team.

    Communicate and explain the discharge plan to patients and their families, addressing any concerns or questions they may have.

    Arrange necessary aftercare services such as home care, physical therapy, medical equipment, or follow-up appointments.

    Work closely with insurance providers to understand patient's coverage and benefits, and to ensure the necessary services are covered.

    Advocate for the patient's needs and preferences, ensuring their voice is heard in decisions about their care.

    Maintain detailed records of patient interactions and discharge plans, ensuring compliance with all relevant regulations and confidentiality requirements. Including data collection for hospital and state programs.

    Provide education and resources to patients and their families to promote health and prevent readmission.

    Qualifications
    • BSN preferred, graduation from an accredited nursing program
    • Current licensure as a Registered Nurse
    • Minimum of 3 years in a hospital setting, preferably in a discharge planning or case management
    • Excellent communication and interpersonal skills
    • Experience in facilitating change through interdiscilpnary collaboration.
    • Ability to handle high-stress situations and make critical decisions quickly
    • Strong organizational skills and attention to detail
    • Recent working knowledge of Interqual criteria and Milliman Care Guidelines preferred.
