    We are looking for a qualified Community Health Nurse to help us expand our services and assist more seniors in need. The ideal candidate will be a responsible and independent professional who is able to provide the best care in a compassionate manner. Within the nursing process framework, the Community health Nurse performs comprehensive assessment and provides intervention, evaluation and/or referral to meet identified client needs. Through collaboration with health professionals and other team members, the Community Health nurse works to implement client care plans and to manage and coordinate continuity of care.

    Hours for this position: up to 35 hours per week

    Salary up to $43 per hour

    Schedule: Mon-Fri

    Qualifications:

    Valid RN license

    Must be fully vaccinated against COVID-19

    Knowledgeable of Infection Control and Prevention

    Must be current on CPR Certification

    Must be able to travel at least up to 20 miles from where you live

    Must have Driver's License with insured, reliable transportation

    Responsibilities:

    • Provide in-home care for clients based on personalized plan of care
    • Oversee caregivers and perform supervisory visits and competency testing
    • Provides physical and psychological support for patients, friends, and families.
    • Promotes patients' independence by establishing patient care goals, teaching and counseling patients, friends, and family to reinforce their understanding of disease, medications, and self-care skills.
    • Maintains patient confidence and protects operations by monitoring confidential information processing.
    • Maintains documentation of patient care services by auditing patient and department records.
    • Provide in-home care for clients based on personalized plan of care
    • Perform and maintain on-going client case management
    • Provide care coordination for individual clients, including coordination with other agencies, healthcare providers, pharmacies, medical supply companies, etc
    • Manage the client's nursing schedule; be prepared to fill shifts urgently if needed
