Bronx Community Health Network ( BCHN) was founded in 1996 to ensure that the socio-economically depressed and medically underserved residents of the Bronx have access to high-quality and comprehensive healthcare services, Bronx Community Health Network (BCHN) is a not-for-profit, community-based organization and Federally Qualified Health Center.

The organization has a sub-recipient agreement with Montefiore Medical Group to provide a wide range of comprehensive, coordinated primary medical, oral, and mental health care and related diagnostic, pharmacy, social support, and enabling services for medically underserved/uninsured residents.

BCHN's approved scope includes 9 Montefiore community health centers and 9 school-based health centers.

BCHN's mission is to improve the health of people and families in the Bronx; provide access to affordable, quality health care, especially for uninsured persons; promote disease prevention, early treatment and healthful lifestyles; and obtain financial and other resources for programs and services.

BCHN's service area includes the entire borough of the Bronx, including all 25 ZIP codes. Within this area, Health Center sites focus on the low-income and underserved populations. BCHN is seeking to hire an Outreach Worker/MOV Driver to join the team.

The driver will help link community members with and/or refer clients to other services, programs, and agencies as needed and promote the importance of social supports and healthy lifestyle choices.

This person will help provide information, education and workshops to community residents who need help managing chronic illnesses including diabetes, heart disease, hypertension (high blood pressure), arthritis, asthma, and cancer, and who need assistance with healthcare and social support services.

The driver will transport staff members as needed from one place to another for events, visits or meetings. The schedule is Monday - Friday from 9:00am to 5:30pm and weekend as needed.


Essential Functions:

  • Drive a large van as needed for BCHN.
  • Ensuring vehicle maintenance and upkeep, including timely reporting to supervisor of any problems with the vehicle (i.e. leaks, oil, break flasher). This person will assist with the overall vehicle maintenance.
  • Completing daily vehicle logs (i.e. Time in use, mileage, vehicle notifications, mechanical or structural issues, incident reporting)
  • Returning vehicle to parking lot with full tank of gas
  • Performing general weekly inspection to ensure lights, appliances, etc. are properly functioning.
  • Convey the purposes and services of BCHN programs to community members, partners and stakeholders.
  • Coordinate health screenings, health fairs, workshops and other outreach events
  • Identify potential partners for mobile van outreach events.
  • Conduct health promotion workshops and presentations at locations and events including health fairs, parent teacher meetings and community and faithbased organizations.
  • Register patients at BCHN outreach events, as needed.
  • Provide exceptional service and/or care to all patients, clients and team members.
  • Build and maintain positive working relationships with community organizations and members.
  • Assist in the collection of program data.
  • Regularly update the outreach team and management.
  • Complete and submit monthly reports in a timely manner.
  • Administer surveys to assess relevant trends in the community so the program may be adapted to meet the continually changing needs of the community.
  • Perform other duties as requested by management.

Qualifications:

  • Valid New York State driver's license and excellent driving record required. Must successfully pass staterequired background and DMV checks.
  • A minimum of High School Diploma or GED
  • 35 years' experience driving a cargo van, bus, or similar vehicles. Reliable individual with knowledge and familiarity with Bronx communities.
  • Computer proficiency required.

Special Skills and Knowledge:

  • Experience working in diverse communities. Ability to work with individuals and in a group and individual settings with people of all cultures.
  • Basic knowledge of chronic disease and/or social support resources
  • Ability to listen to and communicate clearly and respectfully with community members, and partners and other team members.
  • Commitment to working as a member of a team.
  • Flexibility and willingness to work a flexible schedule primarily outdoors, including inclement weather conditions, and evenings/weekends when possible.

Pay:
From $52,000.00 per year
