- Accountable for the selection, supervision, and development of social workers, chaplains and program coordinators assigned. Ensures that staff competencies are completed as required by regulation and that assigned staff adhere to agency standards. Initiates performance improvement plan with staff not meeting these standards.
- Administers and directs all day-to-day operations of the assigned team. Ensures social services provided to patients meet Medicare Conditions of Participation (CoP's), NHPCO guidelines, Lumina Policies and Procedures. Promotes evidence-based quality of care standards.
- Maintains appropriate patient case load in field of practice.
- Assumes accountability for, and oversight of patient care in collaboration with the RN Clinical Manager, Medical Director and hospice physicians.
- Provides leadership to ensure appropriate interdisciplinary communication, collaborative work relationships and a customer-oriented environment responsive to the needs of all external/internal customers.
- Establishes and ensures a safe work environment for staff to prevent injuries from occurring or recurring. Ensures safe practices in delivery of care to patients and adherence to agency standards for work performance and productivity.
- Ensures that services provided are fiscally responsible and within the standards set by the agency and regulatory bodies.
- Using data and analysis, collaborates with an interdisciplinary team for strategic planning, improving outcomes and fiscal health of the agency.
- Contributes to the agency's marketing efforts, recognizing new opportunities and ventures as they arise, assuring viability and growth of the agency.
- Takes an active role in collaborating with staff on all patients from referral to discharge via IDT conference every 2 weeks and as needed.
- Assists staff in their professional and career development and clinical leadership through mentoring and coaching.
- Independently pursues professional and career development opportunities to keep abreast of changes and trends within the hospice field.
- Establishes, coordinates and chairs team meetings for the purpose of education, clinical operations communication and interdisciplinary care conferences.
- Participates in the Administrator On-Call rotation.
- Attends weekly Leadership team meetings.
- Serves as assigned on agency-wide initiatives and/or committees.
- Participates in QAPI activities and outreach.
- Contributes to communication, positive work environment, and team effort.
- Assumes other responsibilities assigned by the Executive Director
- Understands and adheres to Lumina Privacy Policy; Lumina Abuse of Vulnerable Persons policy; and all policies and procedures contained in the Lumina Employee Handbook.
- Perform other duties as assigned.
- Commitment to serving the needs of the terminally ill.
- Qualification and license (if applicable) for the following:
a). Social Work-Full social work licensure in the State of Oregon (LMSW or LCSW). LCSW preferred.
b). Chaplain:
i. A Master of Divinity or equivalent in Theology is preferred.
ii. Must be ordained or commissioned in ministry that is ecclesiastically endorsed by a recognized faith group or religious organization, have at least one unit of Clinical Pastor Education (CPE) and remain in good standing with the jurisdiction of the denomination practiced.
iii. Board Certified Chaplain preferred. - Minimum 5 years increasing experience, preferably including management of hospice or other medical social services teams. Must have a minimum of 5 years of professional level experience in Social Work, Chaplain, or other related discipline. Preferred experience would include duties of performing as lead to project teams or providing direction or supervision to others for some of that time.
- Program development experience.
- Moderate-advanced level functioning in current Windows and Microsoft Office applications and the ability to learn to use medical software at a professional level.
- Flexibility, independence, and demonstrated superior skills working with patients, families, and professional colleagues.
- Must be reliable in meeting timelines and providing information and have the ability to work independently and as a member of an interdisciplinary team.

Lumina Hospice and Palliative Care founded as Benton Hospice Service in 1980, is a community-grown nonprofit, that provides compassionate care and support for individuals, families and caregivers facing serious illness or end of life.
Lumina Hospice & Palliative Care (Lumina) is seeking a full-time (1.0 FTE) Social Services Clinical Manager to join our team. The Social Services Clinical Manager plays a critical role in helping Lumina deliver its mission of providing compassionate care and support to those facing end of life, by being responsible for the clinical, fiscal, and administrative management of the provision of clinical social services (MSW's, chaplains and bereavement program) as well as the volunteer and transitions coordinator. We have an excellent team of caring individuals committed to our mission by sharing their talents in our non-profit environment.
Your responsibilities will be to:
Qualifications:
Schedule: Usual hours fall within the 8-5, Monday through Friday work week although occasional other hours will be required to meet family and staff needs and fluctuating workload.
*Must have full access to an insured vehicle for work with expectation of occasional driving.
**Must maintain an eligible driving record throughout employment.
***Preferred applicant will be asked to pass a background check, DMV check and drug screen following a job offer.
****New employees are required to complete Tuberculosis screening and provide proof of Hepatitis B vaccination or signed declination.
*****Must be able to show proof of COVID-19 vaccination.
