Job Overview:
AHC Home Health and Hospice is looing to hire a dual role RN Case Manager to join our team.

As a Case Manager, you will be responsible for coordinating and managing the care of patients in various healthcare settings.

This is a vital role that requires strong organizational skills, attention to detail, and the ability to work collaboratively with a multidisciplinary team.

Duties:

• Conduct comprehensive patient assessments to determine their needs and develop individualized care plans
• Collaborate with healthcare providers, including physicians, nurses, and therapists, to ensure the delivery of highquality patient care
• Coordinate and facilitate communication between patients, families, and healthcare professionals
• Monitor patient progress and make necessary adjustments to care plans as needed
• Advocate for patients' rights and ensure they receive appropriate services and resources
• Provide education and support to patients and their families regarding their healthcare options
• Utilize managed care principles to optimize patient outcomes and minimize healthcare costs
• Utilize utilization management techniques to ensure appropriate utilization of healthcare services
• Collaborate with insurance companies and other payers to obtain authorization for necessary medical treatments or services

Requirements:

• Nursing degree (required)
• Previous experience in case management or a related role (preferred)
• Excellent communication and interpersonal skills
• Ability to work independently as well as part of a team
• Strong organizational skills with the ability to prioritize tasks effectively

$85, $95,000.00 per year
