The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible.


Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home.

On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services.

***: Under the direction and supervision of the Program Manager, the Center Supervisor provides operational support to the PACE center by directing and supervising the administrative support staff and the center geriatric aides.


DUTIES / RESPONSIBILITIES:


  • Provides supervision, leadership, and direction to the center-based Geriatric Aides (I & II).
  • Manages GA and administrative support staff schedules and implements staffing plan.
  • Under the supervision of the Program Manager, the center supervisor will be responsible for recruiting, orientation and training, disciplinary actions, and terminations, as necessary.
  • Supervises GAs and support staff, promotes staff development, and evaluates staff performance.
  • Promotes employee recognition and exceptional customer service.
  • Ensures that GAs and support staff are effectively performing duties and provides ongoing coaching and training.
  • Oversees time cards, and administrative duties for payroll.
  • This position may supervise additional staff as assigned.
  • May provide supervision, leadership, and direction to additional administrative support staff as identified by the local Program Manager. Tasks include 1(a) through 1(f) as above.
  • Oversees the daily operations of the center

This includes:

  • Participant care
  • Inside appointments
  • Transportation communication and coordination
  • Meals & snacks
  • Supplies
  • Generates reports and completes any required EHR data entry.
  • Serves as an effective member of OLSHS's management team. Collaborates with other supervisors and participates in required meetings.
Works collaboratively with the program manager on issues related to quality programming, efficiency and cost effectiveness, such as:

  • Advance the goals and objectives of the organization
  • Develop department specific performance goals
  • Implement quality improvement plans
  • Participate in establishing and monitoring financial targets.
  • Maintains active involvement in the promotion of On Lok in the community, which may include after-hours events.
  • Participates in the administrator on-call rotation providing after-hours coverage for On Lok in their respective region.
  • Actively participates in disaster planning activities and training (Incident Command System). 10. Other duties as assigned.

QUALIFICATIONS (knowledge, skills, abilities):


  • Bachelor's degree preferably in healthcare or business management or equivalent experience. Health related experience within the last three-years. Additional healthcare related certification/licenses preferred.
  • At least two-years of supervisory experience preferred.
  • Is efficient, responsive, professional, and has excellent organization and communication (verbal and written) skills.
  • At least one-year experience with the frail or elderly.
  • Computer literate with Microsoft Office Suite.
  • Has a demonstrated ability to lead and maintains an upbeat and positive approach to reinforcing behaviors and creating a highly effective team.
  • Skilled facilitator and problem solver with excellent interpersonal skills with both internal and external customers.
  • Ability to work effectively independently and as part of an interdisciplinary team.
  • Bilingual skills in Cantonese or Spanish preferred.
