Job Posting for Comprehensive Community Based Youth Services Case Manager

The Comprehensive Community-Based Youth Services (CCBYS) Case Manager is a full-time position to provide direct services at home and in the community to individuals and families.

Services are guided by Youth Services

Network, Inc. and State policies as directed by the CCBYS Supervisor.


Job Summary:

We seek a Bachelor's level applicant to provide crisis intervention and case management services to identified youth.


Requirements:

• Minimum of a Bachelor's Degree in a human servicerelated field.
• Minimum of one (1) year experience working with children/adolescents.
• Valid driver's license, proof of automobile liability insurance, and access to an automobile to respond to crisis interventions and transport clients.

Skills:

• Proficient in both verbal and written communication.
• Demonstrate problemsolving techniques.
• Ability to exercise discretion and ensure confidentiality.
• Capable of handling highstress situations.
• Works well in a team environment.

Duties/Responsibilities:

• Provide ongoing case management services, including family support, school advocacy, linkage to community agencies, treatment planning, and more.
• Document all contacts with or on behalf of clients and maintain updated case files.
• Complete required assessments and reports as needed.
• On scheduled on-call days, provide 24/7 crisis intervention, family reunification, and stabilization services to clients and their families.
• Transport youth to alternative living arrangements when necessary.
• Become knowledgeable about the Juvenile Court Act and other State mandates that deal with crisis intervention and case management.
• Attend all scheduled staff meetings.
• Maintain confidentiality of all client information.
• Perform other related duties as assigned.

$42, $46,000.00 per year
