SOCIAL SERVICES COORDINATOR - Full-Time - Generations at Regency, Niles, IL

_ Generations at Regency in Niles, Illinois Seeks a Dynamic Social Services Coordinator. This is a Full-Time role with an 8-hour shift, and an __Annual Salary of Between $55k and $60k, depending upon Experience_

30am - 5:
00 pm, with one Sunday rotation per month._

Responsibilities of the Social Services Coordinator:

Assist with planning, developing implementing our facility's social service programs.
Ensure that medical, emotional, and social needs of each resident is met/maintained
Greet and orient new residents
Provide information regarding community resources to residents' families
Assist with discharge planning
Attend regular care plan meetings with residents and their families
Attend intervention, rehabilitation, marketing, performance improvement and Medicare meetings as appropriate

Requirements:

of the Social Services Coordinator:

Experience in the social services field
Demonstrated knowledge of the social dynamic of the geriatric population
Knowledge of government regulations as relevant to social services a plus
Bachelor's Degree or Master's Degree in Social Work (MSW) a plus, but not required
A member of the National Association of Social Workers (NASW) and Academy of Certified Social Workers (ACSW) a plus
Knowledge of community resources helpful
Initiative to seek new methods and services for residents
Strong relationship building skills
High level of organizational professionalism
