    Care Coordinator for Managed Patient Care - Edgewood, United States - University of New Mexico

    Description

    We are looking for a Care Coordinator to join our Managed Patient Care team. This is a full-time role with a day shift. In this role, you'll coordinate all systems and services required to ensure that our managed patients receive patient-centered care. You'll also manage treatment plans, work with physicians and other staff, and monitor patient outcomes. This job may require you to work with patients of all ages.

    Key responsibilities:

    • Identify appropriate patients within designated specialty areas needing patient case management interventions
    • Perform assessments, data collection, and analysis
    • Collaborate with physicians, nurses, and others to develop comprehensive multidisciplinary plans of care, discharge plans, and patient-centered goals
    • Review patient treatment plans with the healthcare team, and intervene when necessary
    • Coordinate and evaluate the use of resources and services in a quality-conscious, cost-effective manner
    • Provide patient and family instruction based on identified learning needs
    • Participate in team meetings and contribute to quality improvement activities

    Qualifications:

    • Graduate of a nationally accredited nursing program
    • Current RN license in the state of hire, or valid license through reciprocity agreement
    • CPR/BLS certification within 30 days of hire
    • At least one year of directly related experience

    We welcome bilingual candidates who speak English and any combination of the following languages: Keres, Tewa, Tiwa, Towa, Zuni, or Navajo.

    Physical demands:

    • Light work, with frequent walking and standing
    • May require the ability to lift up to 20 pounds at times

    Working conditions:

    • Minor hazards present, including dust, noise, and fumes
