    Nurse Practitioner – Family Medicine

    Full Time

    Nurse Practitioner

    The Nurse Practitioner promotes and maintains health by providing medical services to St. Luke's Medical Clinic patients. The Nurse Practitioner identifies short-term and long-term patient care issues that must be addressed, provides information and prescriptions, recommends treatment options and courses of action.

    Position Summary

    With a quality, compassionate, person-centered approach, the Nurse Practitioner will:

    • Provide direct patient care in a Christian based non-profit clinic.
    • Diagnose, Treat, and Manage acute, chronic illnesses and mental health.
    • Instruct and counsel patients and families on emotional problems of daily living, promoting wellness and health maintenance.
    • Provide continuity of care by developing and implementing patient management plans

    · Work collaboratively with patients, families and providers to ensure high quality care

    • Provide "On-Call" coverage as scheduled.
    • Adhere to clinical guidelines, performance of PCMH/QI measures.

    · Comply with federal, state, and local legal and professional requirements

    · Work in collaboration and continuous partnership with providers, clinical staff, and community resources in a team approach to promote access to appropriate care in the goal of enhancing patient health.

    · Provide consultations and care coordination regarding clients including other services related to wellness and other activities designed to provide integrated health care services

    · Perform coding and documentation of services provided in accordance with requirements

    · Maintain the patients comprehensive clinical record through detailed documentation

    Minimum Qualifications:

    • Master's or Doctor's Nurse Practitioner Degree from an accredited college or university
    • License to practice as a Nurse Practitioner in the State of Colorado.
    • Possess active DEA, board certification and CME required for position.
    • 2+ years' experience as a Nurse Practitioner
    • Experience with managing complex patient care in adult populations.

    · Excellent leadership skills

    Current immunizations are required to work at St. Luke's Medical Clinic and may vary dependent upon the position.
