- Master's degree in Nursing and graduate of an accredited Nurse Practitioner program
- Nurse Practitioner Certification by ANCC or AANPCB
- State Board of Nursing unrestricted license or compact practice privileges
- DEA permit and other necessary certifications
- BLS certification from AHA or Red Cross
