A Day in the Life


This Outreach Navigator will act as a Linkage to Care Specialist and will play a pivotal role in facilitating the coordination of initial appointments for HIV/HCV-positive individuals identified through the FOCUS program.

The specialist will work closely with the Department of Health (DOH) to determine patient status and will undergo training to provide HIV/AIDS counseling, testing, and linkage services, all while adhering to confidentiality and privacy policies.

Additionally, the specialist will collaborate with hospital and community resources to offer diverse and equitable linkage options for patients, aiming to optimize they access to healthcare.

The specialist will also be performing data entry, clerical functions, and providing assistance to the Project Director in monthly and quarterly data reporting.


Summary


The Outreach Navigator develops relationships with community partners to provide opportunities for education and outreach to uninsured and underinsured individuals.


The scope of this position includes:

  • Coordinating meetings with providers, community programs and other groups
  • Acting as a resource for consumers
  • Enrolling individuals in insurance programs under the Jackson Insurance Navigator grant.

Responsibilities

  • Coordinates with providers and community groups to schedule Jackson Insurance Navigator at community events, health fairs and others gatherings.
  • Assists Project Director in scheduling assignments for outreach and inreach initiatives.
  • Conducts outreach to consumers with a health insurance and coverage option focus. Distributes fair and impartial information concerning enrollment in QHPs, the availability of premium tax credits and costsharing reductions in accordance with federal tax laws and enrollment in public programs.
  • Educates the public about the value of health insurance for the individual and family in culturally and linguistically appropriate ways.
  • Informs consumers, particularly the uninsured and underinsured, about health insurance and coverage options available through
  • Aids consumers in understanding health insurance terms, such as premiums, deductibles and coinsurance.
  • Provides materials and explanations about the ten required Essential Health Benefits of all Affordable Care Act policies.
  • Explains program eligibility rules for advance premium tax credits, cost sharing reductions, Medicaid or CHIP.
  • Describes the various methods available to purchase and enroll in QHP.
  • Addresses questions regarding access to enrollment methods and the submission of enrollment documentation.
  • Explains the enrollment criteria for purchasing insurance through
  • Administers tools to assess insurance needs for individuals, including all persons who reside in a household, to identify possible eligibility for insurance options.
  • Assists individuals with the enrollment process.
  • Provides referrals to other social service providers or Navigators as needed.
  • Handles postenrollment inquiries issues and grievances.
  • Maintains expertise in Navigator duties and program specifications.
  • Gathers information for verification, statistical and grant reporting purposes.
  • Monitors consumer presence at locations and activities to ensure that outreach efforts in targeted neighborhoods remain viable.
  • Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise). Performs all other related job duties as assigned.

Experience
Generally requires 3 to 5 years of related experience.

Experience in HIV/AIDS or HCV services, patient care, social work, and/or public health preferred.

Spanish bilingual preferred.


Education
High school diploma is required.

Bachelors degree in related field (e.g., Public Health, Social Work, Healthcare Administration) preferred.
