RESPONSIBILITIES
- Conduct initial assessment to determine residentclient's specific needs and create connections to needed program(s), services, and resources, while maintaining strong commitment to client confidentiality
- Develop and maintain relationships with various communitybased agencies, community groups, to obtain services for clients and stay abreast of new trends and practices in community outreach.
- Work a flexible schedule, including availability to work some evenings, and weekends.
- Meet all goals set by funder for Outreach Worker position
- Perform all other duties and tasks as assigned
QUALIFICATIONS
- Minimum high school diploma or GED equivalent credentials is required.
- Experience working with the targeted community or in a similar environment with the population of clients we serve; (preferred)
- Strong use of the English language in verbal and written communication. Bilingual in Spanish preferred.
- Authorization to work in the United States for any employer is mandatory.
PHYSICAL REQUIREMENTS
- Ability to use a computer for prolonged periods.
- Ability to occasionally lift and/or move up to 10 pounds.
- Ability to stand, walk, or sit for long periods of time.
- Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork.
- Ability to bend and retrieve objects and/or documents.
- Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
- Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.
