    Position Summary:


    Rehab Liaison is a highly visable professional member of the Health care team who has 7 day a week accountability for developing Acute Rehab census as defined through targeted goals of the business plan.

    Responsible for developing referral relationships internally and externally with an emphasis on face to face contacts.

    Liaison will assist and manage the coordination of referral to admission conversion process and represent JMH rehab in community related activities.

    The Liaison must have a good understanding of Rehabilitation and associated CMS and CARF regulatory requirements. Position reports to the Director of Rehab admissions at Jackson Memorial Hospital. Must be able to function in fast paced environment.

    Duties & Responsibilities:

    Excellent assessment and critical thinking skills to screen potential Rehab candidates using regulatory and Medicare guideline.

    Meticulous medical record review of physician notes; diagnosis, labs, radiological results; consults surgical, physical/functional, mental, legal, psychosocial, environmental, family support, spiritual, cultural and financial status.

    Provides coverage for weekends on a rotating basis. Rehab certifications within 1 year, CRRN if eligible. Presents cases in daily huddle, coordinates Rehab admission process with Jackson Rehab admission team. Maintains admission productivity goals.

    Attends ID rounds in assigned areas daily and Interfaces with patients / families to answer questions and facilitate transfer as needed.

    Recognizes barriers to admission, responds appropriately and follows up on admission variables to resolve issue. Demonstrates positive referrer satisfaction and account retention by establishing effective physician relationships for referral success. Knowledge of insurance, managed care principles and practices. Discuss cases and provide justification with provider and insurance companies.

    Assists in compiling and inputs statistical information necessary, participates in Rehab Business leadership meetings and performance improvement team activities, maintaining 100% regulatory compliance with preadmission screen signatures.

    Provides in- services, education programs and professional presentation for referral sources, community groups and other organizations regarding Jackson Rehab services, programs and outcomes.

    Performs other related duties as assigned. Demonstrates behaviors of service excellence and CARE values (Compassion, Accountability, Respect and Expertise). Other duties as assigned.

    Qualifications

    Experience:

    3 to 5 years of related experience required

    Education:

    Bachelor's degree in related field is required

    License Certification:

    Licensed as specialized clinician is required.

    Knowledge Skill Abilities:

    Ability to analyze, organize and prioritize work accurately while meeting multiple deadlines. Ability to communicate effectively in both oral and written form. Ability to handle difficult and stressful situations with professional composure. Ability to understand and follow instructions. Ability to exercise sound and independent judgment Knowledge and skill in use of job appropriate technology and software applications.

    Physical Demands:

    Job functions requires to Lift objects weighing up to 20 pounds. Additional information and provision for requests for reasonable accommodation will be provided by each hiring department.
