    Description

    Job Title: Bilingual - RN Care Manager V

    Location: Syracuse, NY (Field position)

    Duration: 6 Months

    Job Description: UAS RNs are needed to conduct assessments in the Syracuse area. Responsibilities include visiting members in various facilities to conduct UAS Assessments. Orientation is provided for UAS certified RNs. Bilingual candidates are preferred.

    Essential Functions/Responsibilities:

    • Conduct assessments of members' needs
    • Develop individual care plans
    • Manage care transitions and discharge planning
    • Collaborate with the care team
    • Analyze data to improve care delivery

    Skills/Aptitudes:

    • Comprehensive clinical assessment skills
    • Excellent communication and computer skills
    • Ability to work independently and in a fast-paced environment
    • Strong problem-solving skills
    • Ability to build relationships with providers and team members

    Summary: Responsible for managing the health care of members to achieve optimal outcomes. Develops and implements integrated care plans, monitors progress, and ensures compliance with regulations.

    Knowledge/Skills/Abilities:

    • Strong interpersonal and organizational skills
    • Knowledge of regulations and coding
    • Experience with case management tools
    • Ability to take initiative and see tasks through completion
    • Computer proficiency and excellent communication skills

    Required Education: Bachelor's Degree in Social Work or Health Education or equivalent experience

    Required Experience: 10+ years of clinical and case management experience

    Required Licensure/Certification: Valid driver's license with good driving record required
