    Job Description

    As a Care Manager at Reflections Management and Care, you will utilize your knowledge and skills in a client-centered approach, providing holistic care management to our clients.

    This role demands flexibility, as it involves transitioning from part-time to full-time based on the growth of your caseload.

    Typical Day Will Look Like:


    Conduct assessments and monitoring, plan and solve problems, and provide education and advocacy to ensure the well-being of our clients.

    Collaborate with families and other healthcare professionals to create and implement personalized care plans.

    Lead efforts in marketing and intake to attract new clients and referrals, showcasing the quality of care Reflections Management and Care provides.

    Use your expertise in navigating healthcare systems to advocate for clients, ensuring they receive the best possible care.

    Position Goals:


    Your primary mission is to improve the quality of life for our clients by providing comprehensive and compassionate care management services.

    Utilize your knowledge in care management, healthcare, or social work to identify and address the unique needs of each client.

    Continuously seek out new ways to enhance care delivery and client satisfaction.
    Interpret and act on performance data to refine and optimize care plans.

    Performance Indicators:
    Effective collaboration with the team and successful management of client care.
    High client and family satisfaction levels.
    Consistent delivery of high-quality care and adherence to deadlines.

    Qualifications:
    Master's in Social Work, RN, or a similar degree with relevant care management experience.

    At least 5 years of experience in senior care, with a preference for those with a background in care management, social work, or hospice care.

    Eligibility for certification by the National Academy of Certified Care Managers is a plus.
    Demonstrable experience in working with individuals with Alzheimer's disease or other forms of dementia.
    Excellent written and oral communication skills, with a proven ability to work independently and as part of a team.
    Must have reliable transportation for client visits and a commitment to maintaining high standards of care and professionalism.

    Requirements:
    A profound understanding of the aging process, including psychosocial issues, medical procedures, and medication management.
    Strong supervisory and interpersonal communication skills, with the ability to train and lead others.
    Knowledge of healthcare systems, medical terminology, and the ability to navigate complex care scenarios.
