Description

Monitors and manages patient care to ensure continuity, optimal outcomes, patient satisfaction, cost efficiency, and compliance.

Reports to:
Director of Case Management

Minimum Education
- Associate's or Bachelor's degree or RN Diploma in Nursing required

Required Skills
- Critical thinking skills, decisive judgment, ability to work autonomously, perform under stress, and take appropriate action

Certifications:
- BLS certification from the American Heart Association (AHA) or American Red Cross (ARC) required within 90 days of hire if not current

Licenses:
- Current Tennessee or Multi-State RN licensure required

Minimum Work Experience
- 3 years experience in acute care nursing or related field required
- Minimum 1 year case management experience in an acute care or critical access hospital preferred
