    Description


    The LPN ChargeNurse is responsible for clinical and administrative tasks in a single homesetting for one individual requiring 24/7 skilled care.

    This young adult isengaged in her community and leads a full lifeâ€"as long as her medical needs aremanaged by a skilled nurse.

    The Charge Nurse is responsible for coordinatingcare with the LPN team at the home, physicians and other community healthcareproviders, and non-clinical Georgia Options staff who assist the individual tolive as independently as possible.

    *This is a Fulltime Monday-Friday Day shift position


    ESSENTIAL FUNCTION AND RESPONSIBILITIES

    • Carry out nursing care according to the careplan
    • Assess for and identify potential healthproblems, report changes in condition
    • Complete and maintain electronicdocumentation of health status
    • Manage, maintain, and administer medicationsand G/J tube feeding
    • Manage medical care with other providers,including staff LPNs
    • Manageequipment, medical supplies, and other specialized needs
    • Advocate forindividual as necessary
    • Serve asliaison for family and Georgia Options
    • Assist Direct Support Professionals toprovide community living support and non-skilled personal care
